The best ripper is Exact Audio Copy, coupled with the LAME MP3 encoder.
On top of being very high-quality, Exact Audio Copy is also able to copy some copy-protected music CD's.
However, it is not the easiest thing for amateurs to set up.

So, if you want to avoid the trouble, CDex is almost as good as Exact Audio Copy, and I host an installer on my site here.
